Rae Ann Williams, M.D., FACP

Rae Ann Williams, M.D., FACP is the Regional Medical Director for Primary Care at HealthPartners Medical Group.  Dr. Williams is an experienced clinician with extensive knowledge in process improvement and clinical operations.  Earlier this year, she transitioned from her longstanding primary care practice to a consultative practice at the Thrombosis Clinic within the HealthPartners care group.  In her current leadership role as a Regional Medical Director, Rae Ann is primarily responsible for the health and patient experience outcomes of the 7 clinics in her region. 

Rae Ann’s interests include systems-based approaches to preventive care delivery, diabetes care optimization, and hypertension management.  In addition, she has participated in work to centralize primary care services including medication refills, anticoagulation management and cervical cancer screening. Through her division’s work on redesigning ambulatory care, HealthPartners has twice been awarded the American Medical Group Association (AMGA) Acclaim Award recognizing innovation in health care. 

Dr. Williams is a 1988 graduate of the University of Minnesota Medical School and completed her residency at Hennepin County Medical Center in Minneapolis.  She is board certified in Internal Medicine and a Fellow of the American College of Physicians.
