Health equity is achieved when every person has the opportunity to “attain his or her full health potential” and no one is “disadvantaged from achieving this potential because of social position or other socially determined circumstances.” AMGA is bringing together Medical groups and health systems on November 7, in Arlington, VA that are working to address and reduce disparities in outcomes, address inequities through identifying the social determinants of health, and promote equitable access among their patient populations and the communities they serve. Our program features interactive panels, breakouts sessions, and group work led by a nationally recognized faculty to learn how health systems are driving change through their health equity initiatives and the unique challenges they face. Chronic Care Roundtable (CCR) Meeting

Industry Leaders Are Making a Difference. Chronic diseases account for the vast majority of health spending: 81% of hospital admissions, 91% of prescriptions filled, and 76% of all physician visits. The Chronic Care Roundtable is AMGA Foundation’s initiative to address these pressing conditions by uniting medical groups, health systems, and industry partners to improve care paths for patients with chronic conditions.

Our 2023 Fall CCR Meeting will be "Tackling the Obesity Epidemic: Strategies for Comorbidities of Diabetes, Cardiovascular and Kidney Disease". We will be preparing medical groups and health systems to ensure equitable access to the next generation of obesity therapies and address obesity comorbidities of diabetes, cardiovascular disease, and kidney disease. Members of the Chronic Care Roundtable are collaborating with AMGA leaders to shape initiatives for improved health outcomes.

Rise to  Immunize™️ (RIZE) is bringing together national immunization experts and participating medical groups for a RIZE Symposium in Arlington, VA on November 9-10. This in-person, interactive meeting consists of two half-days of content—including compelling speakers, panel discussions, and breakout sessions—as well as a reception and dinner to network with your peers. This symposium is the only in-person meeting held specifically for RIZE campaign participants.
