      Type 2 Diabetes Best Practices Learning Collaborative

      AMGA’s Type 2 Diabetes Best Practices Learning Collaborative provides a platform for participating healthcare organizations (HCOs) to improve diabetes control and clinically meaningful weight loss through inclusive, evidence-based care for adult patients with type 2 diabetes and obesity. Member organizations, with support from AMGA and a team of advisors, will aim to prioritize achievement of sustained glucose control and personalized weight management to improve overall health for this high-risk population.

      Through the Type 2 Diabetes Best Practices Learning Collaborative, HCOs will engage in a shared community of learning designed to accelerate system-wide transformation, address clinical inertia, sustain and scale care delivery process that are team-based, equitable, and adaptable across settings.

      Organizations will need to be able to access electronic health record data to track and report measures to AMGA.

      Quantitative Outcome Measures

      1. Among active patients with T2DM, prevalence of obesity by weight class

      2. Among active patients with T2DM and obesity, proportion of patients with hemoglobin A1c <7% AND >9%

      3. Among active patients with T2DM and obesity, proportion of patients who received a T2DM or obesity evidence-based treatment

      4. Among active patients with T2DM and obesity, proportion of patients achieving clinically meaningful bodyweight reduction

      Data may require stratification by patient demographics and social determinants of health AMGA Foundation will provide measurement specifications.
