In 2025 more people will turn 65 than in any other time in history. By 2040, more than 80 million Americans will be over age 65, placing new demands on healthcare delivery.

- How older adults view value and loyalty across industries
- The growing costs of inactivity and falls
- The role of active aging in boosting quality measures
- Strategies to enhance patient satisfaction and Medicare retention
- Lessons from risk-bearing healthcare organizations
Led by leaders in geriatrics, health systems, population health, and patient engagement, this session offers practical approaches to reducing fall risk and advancing evidence-based, patient-centered programs.

From Data to Diagnosis: Clinical Informatics in Colorectal Cancer Screening
Wednesday, December 3, 12:00 pm ET
Sponsored by Exact Sciences
Jeff Arroyo, MD, Associate Medical Director, Clinical Informatics, Alta Med
This session highlights how clinical informatics is driving measurable improvements in colorectal cancer screening at a Federally Qualified Health Center (FQHC). By integrating Cologuard into patient outreach campaigns, centralizing phone follow-up, and streamlining in-clinic orders, the care team has been able to systematically close gaps in screening. As a result, screening rates rose from 46.8% in 2022 to 59.0% in 2024, far outperforming the 2024 national FQHC average of 42.7% and approaching commercial plan benchmarks. Participants will see how data, technology, and workflows come together to create sustainable change while reducing clinic team workload. Attendees will leave with practical lessons to apply in their own organizations, showing that informatics is not just about dashboards, but also about saving lives.
Key Takeaways:
- Understand the role of clinical informatics in improving colorectal cancer screening in ambulatory/FQHC settings
- Recognize how technology-enabled outreach (portal, text, phone, in-clinic workflows) leads to measurable improvements in screening rates
- Compare local performance to national payer and FQHC benchmarks to highlight impact
- Apply lessons learned to other preventive screenings and population health initiatives
- Connect data-driven improvements to patient-centered outcomes, reinforcing that informatics is ultimately about saving lives
