Cardiovascular Risk Optimization Best Practices Learning Collaborative
AMGA Foundation, supported by an independent grant from Lilly, is leading a 21-month initiative focused on helping medical groups develop and implement strategies to develop and implement Lp(a) testing programs that increase the proportion of adults receiving once-in-a-lifetime screening, enabling early identification of elevated cardiovascular risk and supporting evidence-based clinical management.
AMGA’s Cardiovascular Risk Optimization Collaborative is designed for organizations interested in teaming with other high-performing medical groups and health systems to develop tailored quality health outcome based, multi-level strategies to address social drivers and close care gaps in Lp(a) testing and cardiovascular risk management.
Through a shared-learning environment, including webinars, listserv discussions, and in-person and virtual meetings, participating organizations will work together to accelerate systematic change and develop sustainable care processes. Learnings will be disseminated to a broader group of health care organizations through individual best practices infographic summaries at the conclusion of the Collaborative.
