      This session highlights how clinical informatics is driving measurable improvements in colorectal cancer screening at a Federally Qualified Health Center (FQHC). By integrating Cologuard into patient outreach campaigns, centralizing phone follow-up, and streamlining in-clinic orders, the care team has been able to systematically close gaps in screening. As a result, screening rates rose from 46.8% in 2022 to 59.0% in 2024, far outperforming the 2024 national FQHC average of 42.7% and approaching commercial plan benchmarks. Participants will see how data, technology, and workflows come together to create sustainable change while reducing clinic team workload. Attendees will leave with practical lessons to apply in their own organizations, showing that informatics is not just about dashboards, but also about saving lives.
