      AMGA's Rise to Immunize (RIZE) national campaign mobilizes medical groups and health systems to improve routine adult immunization rates through evidence-based care processes, quarterly benchmarking, and shared learning. This poster highlights UW Medicine's interventions and results improving RSV, COVID-19, and Hepatitis B vaccination rates as a top performer in the campaign's first year of tracking these three measures.

      Health System Approaches to Assess and Improve Direct Oral Anticoagulant Adherence in Non-Valvular Atrial Fibrillation

      Three health care organizations implemented multi-level interventions to improve primary and secondary direct oral anticoagulant (DOAC) adherence for patients with non-valvular atrial fibrillation (NVAF). Results demonstrate that multidisciplinary approaches to adherence monitoring are feasible within health systems and can reduce hospitalizations while improving overall patient outcomes.

      Collaborate to Innovate: Peer Approaches to Lung Cancer Screening Quality Improvement

      Five health care organizations partnered with AMGA to improve early detection of lung cancer through multi-level quality improvement interventions targeting screening rates among eligible current and former smokers. Findings highlight variation in screening rates across patient demographics and underscore the importance of EHR infrastructure, provider education, and patient outreach in scaling lung cancer screening programs.

      Mind Matters: Elevating Brain Health in Primary Care

      The AMGA Brain Health in Primary Care Quality Improvement Initiative partnered with four health care organizations to improve early detection and diagnosis of mild cognitive impairment (MCI) and Alzheimer's disease and related dementias (ADRD) in primary care. Sites completed structured root cause analyses and implemented targeted interventions to address multilevel barriers to cognitive screening, specialist referrals, and timely diagnosis.

      Earlier initiation of the HPV vaccine (at ages 9-10 versus 11-12) is associated with increased likelihood of completing the series. This poster shares survey insights from providers and staff at 5 healthcare organizations with demonstrated success in earlier initiation including most impactful interventions and recommendations.

      Anticoagulants used to treat VTE are associated with bleeding and recurrent VTE events secondary to inadequate dosing or non-adherence to treatment plans. These medication outcomes are implicated as a major cause of VTE related hospital readmissions. This poster describes the focused interventions of North Mississippi Health System to improve care transitions for patients with VTE.
