      A retrospective on 2025 with a forward-looking view into 2026. The speaker will explore how health systems responded to policy shifts, lessons learned, and the critical questions organizations must ask as they prepare for the year ahead.

      Build, Borrow and Blend Workshop
      This highly interactive workshop moves beyond translation to reimagine care delivery that truly meets patients where they are. Through small-group exchanges, real-world personas, and rapid brainstorming, participants will share, adapt, and co-create culturally tailored strategies that respond to emerging challenges like chronic conditions, and immunizations.

      Case Study Panel: Impact of Partnering with Community-Based Organizations Beyond Social Referrals

      Learn from health systems partnering with community-based organizations to go beyond referrals and build lasting collaborations. This session highlights approaches to addressing SDOH such as food insecurity, housing, and transportation while leveraging community trust and resources.

      Stratify, Interpret, Act: Why Equity Data is Not Optional & Table Discussion

      This session explores why equity data is indispensable for both value-based care and fee-for-service models. Participants will examine how stratifying data by race, ethnicity, sex, insurance, and other factors, combined with SDOH insights, reveals care gaps and the "why" behind them. The discussion will highlight strategies to act on these insights even in communities with fewer resources, and consider how organizations can continue to deliver equitable care in the face of Medicare/Medicaid cuts and limited measurement requirements.

      Solution Rooms

      The Solution Rooms are dynamic, participant-let sessions designed to provide just-in-time peer support. Each attendee comes prepared with one challenge or idea, and in small groups, participants exchange insights, offer feedback, and co-create practical solutions. This format fosters candid dialogue, fresh perspectives, and real-world takeaways, ensuring everyone leaves with actionable next steps and stronger peer connections.

      This session explores innovative strategies to reduce geographic and racial disparities, particularly in rural and frontier communities. Discussion will highlight creative ways organizations are leveraging telehealth, virtual care, and resource-sharing to expand access while working within constrained budgets.

      Room 2: How to Keep Health Equity Relevant?
       
      With shifting priorities, workforce strain, and fluctuating funding, maintaining organizational focus on equity can be challenging. This session creates space for candid conversation about sustaining momentum, building alignment with leadership, and ensuring equity remains central in the years ahead.

      Breakout Session 2

      Room 1: Family Caregiving as SDOH

      Caregiving significantly impacts health equity, particularly among underserved populations. This session will address the emotional and financial toll on caregivers, opportunities to support them as part of patient care, and ways to strengthen the workforce pipeline in home health and caregiving professions.
      Room 2: Equity UnpluggedA candid, open-dialogue session designed to unpack pressing equity challenges in real time. Without a set agenda, participants will share emerging practices, lessons learned, and innovative ideas - leaving space for honest exchange and collaborative problem solving.
