Stratify, Interpret, Act: Why Equity Data is Not Optional & Table Discussion
This session explores why equity data is indispensable for both value-based care and fee-for-service models. Participants will examine how stratifying data by race, ethnicity, sex, insurance, and other factors, combined with SDOH insights, reveals care gaps and the "why" behind them. The discussion will highlight strategies to act on these insights even in communities with fewer resources, and consider how organizations can continue to deliver equitable care in the face of Medicare/Medicaid cuts and limited measurement requirements.
