AMGA Foundation Launches New Initiative to Improve Evidence-Based Care for Patients with Type 2 Diabetes and Obesity
Alexandria, VA – AMGA Foundation, supported by an independent grant from Lilly, will launch a 29-month initiative focused on helping medical groups and health systems develop and implement sustainable and scalable strategies to improve the number of adult patients ages 18 to 75 with both type 2 diabetes (T2DM) and obesity who receive evidence-based care.
The Type 2 Diabetes Best Practices Learning Collaborative is designed to provide a platform for participating organizations to accelerate systemwide transformation in T2DM and obesity care through the development and implementation of quality improvement projects, best practices sharing, and peer-to-peer learning. Participating organizations will focus on achieving sustained glucose control and personalized weight management for this high-risk patient population.
“More than 90% of adults with type 2 diabetes have obesity, a dual burden that heightens the risk of cardiovascular disease and other serious complications,” said John Kennedy, MD, AMGA Foundation president and AMGA chief medical officer. “Despite clear clinical guidelines, nearly half of individuals with T2DM do not achieve recommended glycemic control, and evidence-based weight management approaches remain underutilized in routine care Through this Collaborative, we will equip healthcare organizations with the tools, data, and peer learning they need to close these care gaps and drive lasting, measurable improvements for patients.”
The following medical groups and health systems were selected to participate in the Collaborative:
- Northwest Primary Care (NWPC)
- UW Health Northern Illinois
- WellSpan Health
- UW Medicine Physicians
- Prevea Health
- Palo Alto Medical Foundation (PAMF)
Participating organizations will have access to a National Advisory Committee, a panel of subject matter and clinical experts who will guide participants in developing their quality improvement initiatives and provide educational content. Healthcare organizations will implement evidence-based interventions addressing clinical inertia, multidisciplinary care team integration, and health equity—with a focus on populations experiencing disparities in diabetes and obesity outcomes.
The Collaborative launched in early 2026.
