Together 2 Goal Concludes After Five Years of Diabetes Improvement; AMGA campaign meets goal and recognizes high-performing groups

Alexandria, VA – Today marks the final day of AMGA’s Together 2 Goal® campaign, a five-year initiative that achieved improved diabetes care for more than 1 million people nationwide. 

Launched in 2016, Together 2 Goal® challenged leading healthcare organizations to work together to transform diabetes care for Americans across the country. More than 150 medical groups and health systems joined the effort, implementing one or more evidence-based care processes (called “campaign planks”) to drive improvement. Campaign participants reported data on a quarterly basis to measure progress and utilized Together 2 Goal® resources and tools to advance their efforts. To share lessons learned, organizations leveraged monthly campaign webinars, a listserv, case studies, and a peer-to-peer mentorship program.

As part of the campaign’s final close-out activities, the campaign also recognized its 2021 Together 2 Goal® award winners—three high-performing medical groups and health systems that, along with fellow AMGA member organizations, helped Together 2 Goal® exceed its goal of 1 million lives improved. Ballad Health Medical Associates, New West Physicians, P.C., and Valley Medical Group received this year’s final awards for their significant engagement with the campaign and high performance on the campaign measures. Over the course of Together 2 Goal®, these organizations have improved outcomes for their patient populations, resulting in more people with safer ranges for critical diabetes measures, like blood sugar and blood pressure.

Originally scheduled to conclude in 2019, the Together 2 Goal® campaign extended the initiative to run through March 2021. This additional time provided AMGA members the opportunity to hardwire improvements within their organizations, supporting the sustainability of their efforts beyond the campaign’s conclusion. AMGA Foundation will launch its third national campaign, on a topic yet to be announced, in the fall of 2021. 

The corporate collaborators for the campaign’s extension phase include The Janssen Pharmaceuticals Companies of Johnson & Johnson (Presenting Corporate Collaborator), Novo Nordisk, Inc. (Founding Corporate Collaborator), Optum (Distinguished Data & Analytics Corporate Collaborator), and Merck (Contributing Corporate Collaborator). Campaign partners and supporting organizations include: American Association of Clinical Endocrinologists, American Association of Diabetes Educators, American College of Physicians, American Diabetes Association, American Kidney Fund, Behavioral Diabetes Institute, DiabetesSisters, Endocrine Society, HealthyWomen, Know Diabetes by HeartTM, Men’s Health Network, National Kidney Fund, and National Minority Cardiovascular Alliance.
