1.     AMGA Foundation:  MaineHealth has been a participant in our Osteoporosis Best Practices Learning Collaborative.  How has participating in AMGA population health initiatives directly improved the care Maine Health has provided to its patients?

Participation in the Osteoporosis Best Practices Learning Collaborative has enabled us to focus more time and resources on patients with osteoporosis, improving care and reducing costs. .  This is a relatively small population of our patients, but quite an important one as reducing the re-fracture rates not only improves clinical care but also significantly reduces cost of care.  Through this Collaborative, we built new reporting resources in Epic to meet the need for data sharing and this allowed us to easily track performance on DEXA rates, diagnosis rates and treatment pre- and post-fracture rates.  Using this data, we then highlighted opportunities for improvement and focused on post-fracture treatment, particularly in women.  We are now developing a navigator function for post-fracture patients, and are hopeful this will help us move the needle.  The strategies and tactics we learned from others in the collaborative, along with being held accountable for outcomes has been transforming.

2.     AMGA Foundation:  MaineHealth supported our Campaign for a Healthier America and AMGA Foundation’s vision to improve care for 100 million patients.  Why was it important to Maine Health to invest in the Campaign?

The Campaign for a Healthier America is so important because it recognizes the need to effectively manage chronic medical conditions.  Our patients and communities are dealing with more complex conditions at a time when healthcare access can be a challenge.  The sharing of expertise and best practices with support in implementing evidence based clinical guidelines AMGA Foundation’s population health initiative provide  make an enormous impact on the lives of our patients.-That’s an investment we’re glad to make.

3.     AMGA Foundation: Why would you encourage peer organizations to donate and become involved in AMGA Foundation programs?

Supporting the AMGA Foundation allows their critical population health initiatives to expand, reaching more patients and impacting millions more lives.  The Foundation has the ability to conduct this work on a very large scale, supporting care teams with expertise and resources that they may not otherwise have access to.  This work is in complete alignment with MaineHealth’s vision to make our communities the healthiest in America.
