      Portland Regional Meeting

      Free for medical group members, regional meetings are specifically designed to be interactive with an agenda crafted to facilitate networking among attendees. Time is split between succinct presentations, small group breakouts, and an open floor ("Hot Topics") where attendees can consult with the group for advice on any topic. To ensure the content is specific to individual markets, topics and speakers are identified via a planning committee of AMGA medical group members in the region.

      Transitions of Care: Strategies for Success

      Speaker: Tom Gragnola, MD, FACP, Senior Medical Director, Optum Care Network of Oregon

      The presentation will begin with a discussion of the background and significance of transitions of care, emphasizing the importance for patients, providers and the healthcare community. Will spend time defining and differentiating the components of the transitional care management (TCM) billing and the HEDIS quality measure (TRC) tied to these visits. Then will highlight goals and provide examples of strategic workflows to consistently deliver effective transitions of care.

      Growing Virtual Care through Patient Empowerment & Provider Engagement

      Speaker: Kai Neander, Director, Digital and Virtual Health, EvergreenHealth Medical Group

      EvergreenHealth has increased virtual care year over year since 2021 including through an EMR migration. Focusing on telemedicine, e-visits, and e-consults EvergreenHealth Medical Group has increased access, improved patient satisfaction, and built the foundation for a hybrid care delivery model. Through empowering patients to choose care modalities to restructuring provider templates EvergreenHealth's Director of Innovation & Digital Health will share the successes and challenges in their journey towards building virtual care infrastructure that increases efficiency, reduces burnout, and removes barriers to care.
