The presentation will begin with a discussion of the background and significance of transitions of care, emphasizing the importance for patients, providers and the healthcare community. Will spend time defining and differentiating the components of the transitional care management (TCM) billing and the HEDIS quality measure (TRC) tied to these visits. Then will highlight goals and provide examples of strategic workflows to consistently deliver effective transitions of care.

Focusing on telemedicine, e-visits, and e-consults EvergreenHealth Medical Group has increased access, improved patient satisfaction, and built the foundation for a hybrid care delivery model. Through empowering patients to choose care modalities to restructuring provider templates EvergreenHealth's Director of Innovation & Digital Health will share the successes and challenges in their journey towards building virtual care infrastructure that increases efficiency, reduces burnout, and removes barriers to care.
