Implemented Intervention

Patient Education Strategies: Developed a standardized colorectal cancer screening education handout that was provided to each Primary Care clinic for front desk to give patients at check-in with group meeting goal 95.65%. Same handout pushed through portal for patients with care gaps. Added handout in imbedded link for Epic after-visit summary. Population health and care coordination staff utilized handout during patient encounters.

For those patients who need more than a “reminder,” utilize trained clinical staff to have conversations with those patients to provide greater detail on risks, answer questions, and promote screening.

Implemented Intervention

Targeted APRN Outreach and Orders: Available APRN relevant to the patient’s care conducts outbound phone calls and places Cologuard orders for patients who have not yet had colorectal cancer screening. Patients are typically more amenable to screening at the end of the year.
