“QulC” Tip:
Documentation education is essential to gather data from the EMR systems. If the data is not in a discrete field, it is almost impossible to grab.

Planned Intervention
Our goal is to address abnormal FOB/FIT DNA test to ensure follow-up colonoscopies were completed. Both HEDIS and CMS only measure if the screening was completed and not the results, so most reporting and software solutions do not include results. We also had to restrict the project to our commercial ACO payer to ensure we have information regarding if the follow-up colonoscopy was completed. So we are still in progress of gathering data.
