10 Healthcare Organizations Commit to Further Advancing Type 2 Diabetes Care

Alexandria, VA – AMGA, with support from AMGA Foundation and Novo Nordisk Inc., announced the 10 organizations participating in its Together 2 Goal® (T2G) Diabetes Bundle Best Practices Learning Collaborative, an intensive learning collaborative meant to help T2G campaign enrollees improve the quality of care they deliver to patients with Type 2 diabetes. The participants will endeavor to improve outcomes on the T2G diabetes bundle measure which includes HbA1c control, blood pressure control, lipid management, medical attention for nephropathy.

The following AMGA member medical groups and health systems have been selected to participate in the collaborative:

  • Summit Medical Group, Berkeley Heights, NJ
  • Mercy East Communities, Washington, MO
  • Valley Medical Group, Paramus, NJ
  • Privia Health, Arlington, VA
  • Norton Medical Group, Louisville, KY
  • Ochsner Health System, New Orleans, LA
  • Palo Alto Medical Foundation, Mountain View, CA
  • Geisinger Health, Danville, PA
  • Michigan Medicine, Ann Arbor, MI
  • Excela Health, Greensburg, PA

“Like all of our T2G campaign participants, these 10 organizations are unremittingly committed to improving the way they care for patients with Type 2 diabetes,” said John Kennedy, M.D., AMGA chief medical officer and AMGA Foundation president. “AMGA collaboratives serve as catalysts for the free flow of ideas and discoveries, and the exchange of information, strategies, and support among groups with a shared mission can bring about substantive change. We look forward to helping these participants truly transform care for patients with diabetes.”

This collaborative is an expansion of AMGA’s recent six-month pilot initiative to improve outcomes on the T2G Core Track diabetes bundle measure. During the pilot, the rate of improvement for bundle measures among pilot groups was twice that of other groups participating in the T2G campaign.

The Diabetes Bundle Collaborative offers participating organizations an opportunity to expand their quality improvement work and accelerate their pace of improvement. It also will provide these 10 participants with access to peer organizations, an advisory committee of clinical experts, and AMGA staff to help improve their diabetes bundle measure as well as provide motivation and resources.

Participating organizations will meet in-person twice over the course of the collaborative and will attend bi-monthly educational webinars to discuss insights, plans, challenges, lessons learned, achievements, and new tools and resources. The first in-person meeting will occur in San Diego, CA, on May 8-9, 2019.
