Achieving the Quadruple Aim Through AMGA’s HPPE Program

Improving population health, reducing cost of care, improving care team well-being, and enhancing the patient experience each require the physician enterprise to operate at an optimal level of performance across the entire organization. In this session, our panel of presenters will share early insights from current participants in AMGA’s High Performing Physician Enterprise (HPPE) Program. They will specifically discuss the seven domains that drive high performance in medical groups and how this program can assess performance, guide organizations to course-correct where needed, and ultimately, get us closer to achieving the Quadruple Aim.

A Commitment to Health Equity: Use of Artificial Intelligence to Reach Marginalized Communities

Hear how CommonSpirit leveraged virtual patient navigators and an artificial intelligence (AI) product to conduct outreach in areas with some of the lowest national vaccination rates. Navigators connected with more than 20,000 patients—mostly in their preferred language—to encourage vaccination or determine their status. AI was integral to this successful vaccination campaign.

Enhancing the Medicare Advantage Experience Through Collaboration

Through a collaborative partnership with Humana, The Vancouver Clinic has established care sites to bring resources closer to older adult patients with complicated health concerns. Integrated care teams are thus enabled to take a holistic approach to primary care within this Medicare Advantage model to achieve better outcomes and improved patient experience. Value-based care remains a moving target, and leaders must evaluate and innovate their technology
strategies—including EMR partnerships, third-party solutions, and bespoke technology—to meet the needs of patients, physicians, clinical teams, payers, employers, and communities. Speakers from Privia Health will share their value-based care journey, where they learned there isn’t one game plan that leads to clinical and financial success at scale. They will also share their deployment of a series of micro strategies designed for the unique payer programs, as well as the geographies and patient profiles served by each.

Navigating Precision Health with Population Genomics

Advances in genomics allow precision health to transform the healthcare experience by improving patient outcomes and increasing the efficiency of healthcare delivery.
