Cost

  • The base fee for the survey is based on the number of providers surveyed.  As seen in the table below, the base fee for AMGA members is half that of nonmembers.  To receive this lower rate, a participating group must be in good membership standing with AMGA at the time of the issuance of the final report.

  • Up to five custom questions can be added for an additional $100 per question for paper surveys or $150 per question for web surveys

In addition to the base fees above, the web survey method will incur a charge of $5 per provider. To get an estimate of your base and web survey fees, click here.

  • Per provider charge of $5.00

  • $150 per additional email notice sent to providers in excess of the standard three (Optional)

Other possible charges include (all are optional):

  • $250 survey modification fee (e.g., to change the standard ordering of survey items)

  • If using the web survey, $150 per additional email notice sent to providers in excess of the standard three

  • $300 fee for extra printed copies of the final report

  • For special analyses, fees of $800 for the first four hours of work and $200 for every hour of work thereafter
