Together 2 Goal Participants Achieve Improved Diabetes Care for More than 1 Million People Nationwide

Alexandria, VA – AMGA’s Together 2 Goal® campaign announced its participating organizations have achieved improved diabetes care for more than 1 million people nationwide. The campaign, which is supported by AMGA Foundation, has officially met and exceeded its goal of 1 million lives improved.

Launched in 2016, Together 2 Goal® challenged leading healthcare organizations to work together to transform diabetes care for Americans across the country. To date, more than 150 medical groups and health systems have joined the effort and improved outcomes for their patient populations, resulting in more people with safer ranges for critical diabetes measures like blood sugar and blood pressure.

“Groups’ commitment to this initiative and willingness to share with and learn from their peers is what made this remarkable accomplishment possible,” said John Kennedy, M.D., AMGA chief medical officer and AMGA Foundation president. “That collaboration is the hallmark of the Together 2 Goal® campaign—and all AMGA quality improvement programs—and has helped us move the needle in combatting one of the nation’s most pressing chronic conditions.”

Medical groups and health systems in Together 2 Goal® implement one or more evidence-based care processes (called “campaign planks”) to drive improvement. Campaign participants report data on a quarterly basis to measure progress and utilize Together 2 Goal® resources and tools to advance their efforts. To share lessons learned, organizations leverage monthly campaign webinars, a listserv, case studies, and a peer-to-peer mentorship program.

Originally scheduled to conclude in 2019, the Together 2 Goal® campaign announced in April that it would extend the initiative to run through March 2021. This additional time will provide the opportunity for existing participants to hardwire improvements and for new groups to learn from the great work that’s already been done. The campaign also will provide AMGA members with new and enhanced resources to facilitate additional shared learning and support sustainability efforts.

The corporate collaborators for the campaign’s extension phase include The Janssen Pharmaceuticals Companies of Johnson & Johnson (Presenting Corporate Collaborator), Novo Nordisk, Inc. (Founding Corporate Collaborator), Optum (Distinguished Data & Analytics Corporate Collaborator), and Merck (Contributing Corporate Collaborator). Campaign partners and supporting organizations include: American Association of Clinical Endocrinologists, American Association of Diabetes Educators, American College of Physicians, American Diabetes Association, American Kidney Fund, Behavioral Diabetes Institute, DiabetesSisters, Endocrine Society, HealthyWomen, Know Diabetes by HeartTM, Men’s Health Network, National Kidney Fund, and National Minority Cardiovascular Alliance.
