CHRISTUS Trinity Clinic Receives 2018 Acclaim Award
Two Additional Groups Honored for Progress toward Achieving IOM Aims and Becoming High-Performing Health Systems

ALEXANDRIA, VA — AMGA today announced that the recipient of the 2018 Acclaim Award is CHRISTUS Trinity Clinic, based in Tyler, TX. The Acclaim Award, supported by AMGA Foundation, the association’s nonprofit arm, is designed to recognize and celebrate the successes that medical groups and other organized systems of care have achieved in improving the value—the quality and cost of care—of the healthcare services they provide to their communities. Two additional organizations, Michigan Medicine and Wellmont Medical Associates, were announced as Acclaim Award honorees.

“The Acclaim Award celebrates our members’ innovative work and shares their successes with the healthcare community in order to better serve all patients,” said John W. Kennedy, M.D., president, AMGA Foundation and chief medical officer, AMGA. “The outstanding groups honored with this award exemplify the highest quality health care in the United States. Recipients and honorees are evaluated using our High Performing Health System™ attributes, which result in coordinated, patient-centered, value-based care. We believe recognizing these innovators will help others learn of their work and be inspired to implement similar programs in their organizations.”

CHRISTUS Trinity Clinic
CHRISTUS Trinity Clinic is a not-for-profit multispecialty practice founded in 1995 and is part of CHRISTUS Health’s Northeast Texas Region, which also includes CHRISTUS Trinity Mother Frances Health System, CHRISTUS Good Shepherd Health System, and CHRISTUS St. Michael Health System. The group of distinguished providers includes more than 650 physicians and advanced practice professionals with over 41 specialties providing a world-class continuum of care to patients in 82 locations across an area that would measure as the country's 40th largest state. Over the past two-and-a-half years, they have experienced exponential growth, nearly doubling the number of providers, while remaining true to their mission and serving their communities by meeting the Institute of Medicine Aims for Improvement.

It guides our choices and direction in all that we do,” said Scott Smith, M.D., senior vice president and institute chair of primary care for CHRISTUS Trinity Clinic. “Our mission and our values are who we are as a system―and our strategy is how we enact that mission. Our strategy is to make our communities healthier and help people gain access to exceptional health care as conveniently as possible by collecting and acting on significant data to connect them to the services they need and the providers to deliver it.”

CHRISTUS Trinity Clinic provides care to nearly 200,000 patients annually regardless of psychosocial or socioeconomic status. In addition, five rural health clinics provide primary care services for Medicaid and Medicare patients in rural communities. Last fiscal year, they served approximately 48,000 patients in these medically underserved areas. In 2017, they performed over 1,250,000 ambulatory clinic visits.

Michigan Medicine
Michigan Medicine is one of the largest health systems in Michigan and one of the nation’s leading academic medical centers. The organization employs more than 2,700 physicians across 19 clinical specialties, including primary care. It provides both inpatient and outpatient care through three hospitals (adult, pediatric, and women’s) with 1,000 inpatient beds, and 40 ambulatory locations with 140 clinics, specialty centers for cancer, depression, and cardiovascular care, and home care services. In 2016, the organization had approximately 50,000 admissions, 100,000 emergency room visits, and 2.3 million ambulatory visits. The system also trains more than 1,200 house-officers and 700 medical students.

Wellmont Medical Associates
Wellmont Medical Associates (WMA), also a 2014 Acclaim Honoree, has 250 providers with 135 physicians and a combined leadership team of nine corporate and 16 regional administrative staff. Their region, which consists of urban and rural communities in the Appalachian Mountain area, is an area with a 17.5% poverty rate, a mean family income of $27,000 and a ranking as the third least healthy area in the United States (Gallup-Healthways Well-Being Index: April, 2013). Simply delivering meaningful health care is a challenge. However, improving regional health care is a calling the group believes is the key to improving the overall regional economic dynamic.
