Submitting your Data
Completed questionnaires are to be emailed directly to Brad Vaudrey of Sullivan Cotter at brad.vaudrey@sullivancotter.com. Sullivan Cotter* will maintain a confidential file for each group’s response. No other organization will have access to your group’s individual data.

2012 Survey Deadline
Please complete and submit your data by no later than Saturday, March 31, 2012.
