AMGA’s Transforming Systems of Care Delivery: Learning Collaborative and Knowledge Exchange was initiated in 2004 to focus on system-wide transformational changes that improve care delivery, outcomes, and patient satisfaction, rather than on management of a specific disease. Designed for medical groups that wish to move their organizations closer to the "ideal" healthcare system described by the IOM's Crossing the Quality Chasm, its focus was moving beyond a project mentality to realize enterprise-wide change.
