
The American Medical Group Association’s Best Practices Learning Collaborative is for organizations that strive to improve the care of patients with hypertension. Medical groups, IPAs, academic practices, and integrated delivery systems interested in expanding an existing program or initiating a new program focused on the management of hypertension in their chronic care models were encouraged to submit a case study.
The learning collaborative will commence in November 2008 and conclude in October 2009. The collaborative is supported through conference calls, online resources, consultation with experts in the field, and two in person meetings. All participants will share their experiences, materials, tools, and results with all involved in the collaborative as part of the project.
Ten medical groups have been awarded $20,000 grants (over a two-year period) to support their initiatives; an additional 2 organizations are supported with travel stipends to attend the collaborative meetings.
