Best Practices in Diabetes Care
Compendium
AMGA partnered with sanofi-aventis to develop the Best Practices in
Diabetes Care Compendium. The Compendium was published in March 2006 and
distributed to all AMGA medical groups.

Learning Collaborative
The Best Practices in Diabetes Care Learning Collaborative supports
improvements in the management of diabetes in the ambulatory health care
setting. Five medical groups were awarded $10,000 educational grants to
support their initiatives and the additional five groups were supported
with travel stipends to attend the collaborative meeting.
The learning collaborative commenced March 2006 and concluded in March 2007. The collaborative was supported through conference calls, online resources, consultation with experts in the field, and two in person meetings. All participants shared their experiences, materials, tools, and results with all involved in the collaborative as part of the project.

Symposium
The Diabetes Best Practices Symposium was a two-day shared learning
program for AMGA members that have been able to document improvement in
clinical outcomes, service and efficiency related to diabetes.
Participants presented an overview of their efforts to improve diabetes
care, discuss successes and challenges, and take part in problem solving
discussions with peers in similar organizations.

Diabetes Data Collection Instruments and Protocols
AMGA National Summit on Quality Accountability in Diabetes
To identify best practices for diabetes management, AMGA hosted a
series of webcasts and meetings in 2008-2009. The goals of the summit
were to bring stakeholders together to discuss strategies for delivering
quality diabetic patient care, and to serve as a launching vehicle for
AMGA programs to advance the alignment of stakeholders.
