Send in registration form and provider list.
To enroll in the program, complete and return AMGA’s
survey registration form. Also, send us a roster that lists
all participating providers. If this is your first time
participating, contact AMGA and we will send you a template
that lists all of the data elements (e.g., site, specialty)
that must be reported for each provider. If you have
participated in the past, contact AMGA and we will send you
the profile information we have on file so you can update
that list. We will use your final roster to generate survey
forms for each of your providers.

Start survey process.
This is the absolute latest date you should start. We
encourage you to start earlier, particularly if you have
some providers who will need extra time to reach the minimum
requirement (for benchmarking purposes) of 30 surveys.

Receive reports.
AMGA will update benchmarks with the recent data
submissions and print the standard report , including
individual provider, site and specialty reports for your
group. Custom analysis is handled separately; contact AMGA
to make arrangements.
