AMGA ’s Provider Satisfaction Benchmarking Program makes it easy
for your group to participate.

Step 1: Contact AMGA to receive start up materials
- Choose the survey cycle that is best for your organization.
- Start preparing your provider list
- Decide if you want to use the paper or on-line version of the
survey
- Add up to five extra questions (in Likert-scale format) to the
survey for an extra fee
Step 2: Submit a list of providers to AMGA
This list should include each provider’s:
- Name
- Specialty
- Education (MD, PA, RN, etc.)
- Provider ID Number (assigned by you)
- Site Name
- Date of Birth
- Gender
- Email address (if using electronic survey)
Step 3: Submit a survey registration form to AMGA
The registration form is a 2 page instrument used to collect basic
information about a clinic that wants to participate in one of AMGA’s
surveys.
Step 4: Choose to administer the paper survey using AMGA ’s
four-stage mailing process
The paper survey process is broken down into four simple stages and
typically yields a 60 percent or higher provider response rate.
Providers return their surveys directly to AMGA in postage-paid return
envelopes. The four stages include the:
- Pre-notification Letter
- Survey Mailing
- Thank You Note
- Letter to Non-Respondents

Choose to administer the survey using AMGA’s new web-based survey
method
AMGA has contracted with a vendor to make the survey available to
providers via the web. Should you choose this approach, the vendor
will send each provider a link to the website containing the survey
(along with the provider’s ID code and a website password). The
provider can then complete the survey online at their own leisure. The
web-based survey tool is easy to navigate and complete. In addition,
the vendor will electronically send all ‘thank you’ and ‘reminder’
notices, leaving only the initial pre-notification letter for the
clinic to send out to its providers. Moreover, you can add up to five
extra questions for a fee, just as you can with the paper survey.
Please note: your clinic must choose either the paper survey
or the web-based survey option for all providers.
Step 5: Receive a Comprehensive Report
- Report contains your complete results, including best practices
and national and specialty norms
- Individual provider results remain confidential
- Only sites and departments with three or more provider surveys
reported
- Report is received within six weeks of the survey due date,
three months after submitting your list of providers to AMGA
