2008 Best Practices in Managing Hypertension Learning Collaborative

The American Medical Group Association is pleased to announce a Best Practices Learning Collaborative for organizations that are striving to improve the care of patients with hypertension. Medical groups, IPAs, academic practices, and integrated delivery systems interested in expanding an existing program or initiating a new program focused on the management of hypertension in their chronic care models are encouraged to submit a case study.

Up to 20 groups will be invited to participate in the learning collaborative; 10 will receive $20,000 grants (over a two-year period) to support their initiatives; an additional 10 organizations will receive travel stipends to attend the collaborative meetings. All organizations receiving grants as well as ANY provider organizations conducting independent projects can request to be a part of the learning collaborative provided their projects meet the basic criteria.

An expert panel of judges will review each case study submitted and select those that merit publication as a Best Practice. The judges will be looking for hypertension programs from small, medium, and large practices that meet the following criteria:

  • Care provided in an outpatient setting
  • Patient care is coordinated across the continuum of services
  • Program results in significant improvements in patient health
  • Program engages the entire care team (doctors including generalists and specialists, nurses, administrative staff, pharmacy, patients, etc.)
  • Program that allows for efficient, ongoing measurement and reporting
  • Project design is practical and addresses program goals
  • Intervention(s) are transferrable to other practice settings

Applications are due June 30, 2008. Case studies can be submitted electronically to hypertension@amga.org.

Hypertension Learning Collaborative 2006

Five medical groups have been awarded $10,000 educational grants to support their initiatives and the additional five groups were supported with travel stipends to attend the collaborative meeting.
