Best Practice in Diabetes Learning Collaborative

The Best Practices in Diabetes Care Learning Collaborative supports improvements in the management of diabetes in the ambulatory health cares setting. Five medical groups were awarded $10,000 educational grants to support their initiatives and the additional five groups were supported with travel stipends to attend the collaborative meeting.

The learning collaborative commenced March 2006 and concluded in March 2007. The collaborative was supported through conference calls, online resources, consultation with experts in the field, and two in person meetings. All participants shared their experiences, materials, tools, and results with all involved in the collaborative as part of the project.
