Become a Founding Member: Based on the size of your organization, a financial contribution at Ask Level over three years will qualify your organization as a Founding Member. As such, you will be exclusively recognized as a leader in promoting a transformation in the nation’s health care delivery system.

• Less than 50 FTE physicians $5,000
• 50-149 FTE physicians $7,500
• 150-249 FTE physicians $10,000
• 250-299 FTE physicians $15,000
• 300-499 FTE physicians $20,000
• 500 or more FTE physicians $25,000
