Best Practices in Diabetes Care Compendium and Learning Collaborative: The mission of this initiative is to support and encourage medical groups in their efforts to improve the quality of care for patients living with diabetes. Medical groups that develop innovative, low-cost and efficacious treatments that positively affect the delivery of quality patient care and the management of the bottom line will then disseminate their proven strategies and processes to other medical groups and the healthcare community.

Best Practices in Managing Hypertension Compendium and Learning Collaborative: This project is designed to produce and share tested tools and strategies that most effectively improve the management of hypertension and then share them with other physician groups nationwide in a compendium of best practices to advance the quality of care. Participating project teams will be able to regularly network with peers, participate in bi-monthly conference calls on relevant project topics, and have timely access to industry experts.

Transforming Systems of Care: Learning Collaborative and Knowledge Exchange: The Best Practices in Service Quality shared learning collaborative will bring together medical group practices and organized systems of care that are dedicated to designing systems of care from the viewpoint of the person who the system exists to help…. the patient. The goal of the collaborative is to develop, test, and disseminate best practices that measurably improve patient access, patient-provider communication, and patient-centeredness.
