Breaking the Cycle: Reducing Rehospitalization for CHF Patients

This interactive session will describe how a multidisciplinary
team at Abbott Northwestern Hospital in Minneapolis has reduced the
number of rehospitalizations for patients with a principal diagnosis
of HF within 30 days of discharge by 30 percent in an area of the
country known to have lower readmission rates than the US average.
The changes put in place build on models put forward by Institute
for Healthcare Improvement’s Five Million Lives campaign and what is
known about transitions from hospital to home. This project involved
rapid testing of ideas and involvement of all members of the care
team, including primary care.
The hospitalist and outpatient clinicians are a part of the Allina
Medical Clinic. Drs. Bergeson and Dickey will share key lessons
learned about the involvement of hospitalists to make transitions
successful.

Creation of a New Primary Care
Delivery Model Through Lean Transformation in a System that Fosters
Innovation and Continuous Improvement

ThedaCare Physicians’ “New Delivery Model” initiative grew out the
implementation of Lean production principles, which led to critical
changes throughout the organization. The group strategically
initiated a comprehensive redesign of the primary care office flow
by implementing a New Delivery Model of care that radically changed
the patients experience from the initial call for an appointment,
through the office visit, and continuing on through follow-up care.
The results associated with the redesigned office visit were most
notable in the areas of safety, quality, access,
patient/provider/staff satisfaction, timelines of results, and
productivity.

Measuring E-Prescribing to Sustain Adoption and Support P4P

The Henry Ford Medical Group rolled out e-prescribing to more
than 700 doctors in just over two years with gains in safety,
improvements in generic use rate,
and realized efficiencies in workflow. This session describes how
the Henry Ford Medical Group measures the use of e-prescribing
both to continue to guide adoption and to measure use for pay for
performance.

Improving Quality and Reducing Cost for 30,000 Patients: The
Medical Home and Health Navigator

Officer, Geisinger Health System
Advanced Medical Home has been proposed for decades and recently
has moved to the forefront in the discussion as a means to control
the explosive cost of medical care in an era of economic challenge
and record budget deficits. Drs. Graf and Bloom will describe how
two years of experience using a primary care medical home model,
Proven Health Navigator, allowed Geisinger to review how to develop,
implement, and expand a 21st century care model, resulting in
enhanced provider compensation and satisfaction, improved patient
loyalty, improved comprehensive quality measures, and reduced costs.

efforts are on parallel tracks, leading to the perception that these
two goals are not interconnected. The Sharp Experience is the
group’s quest to become the best place to work, the best place to
practice medicine, and the best place to receive care. This
presentation will demonstrate Sharp Rees-Stealy’s journey by
highlighting how they embed objective measurement, accountability,
and behavioral change into execution of this strategy. Leadership at
Sharp Rees-Stealy Medical group has determined that these aims
are synergistic, and combining successful tactics and processes best
promotes excellence in the healthcare delivery system.

Outreach Programs

Management, Aurora Health Care
Dr. Smith will share Aurora Medical Group’s strategy to enhance
their care management, medical home efforts through multiple
interventions, emphasizing a recent intervention using automated,
proactive patient outreach. The presentation will cover common
barriers practices encounter, the structure of the Aurora program,
how leadership prepared the practice and patients, outcomes,
and lessons learned.

patient experience, and clinical care. Drs. Averbeck and Nelson will
describe how they engage physicians and local leaders in improvement
approaches that involve care teams within their sites.
