Physician Led Risk Contracting Facilitates Improved Healthcare Quality and Value for Patients

Healthcare reform is being pushed through legislative, regulatory, and market based initiatives. One of the strongest mechanisms to generate improvement in the value of patient care is for providers to implement a well-designed, incentive-driven $PMPM contract with a major payer. This is s daunting undertaking for which most medical groups are not prepared.

This presentation will describe the market dynamics and analysis that led a large academic group practice and associated private practice IPA to undertake such a model with the largest insurer in the state, Blue Cross Blue Shield of MA. For a variety of reasons the academic Medical Center did not join the risk bearing part of this contract. We will discuss the details of how physician groups can take the lead in changing local market dynamics through innovative at risk contracting even when hospital partners may not be ready, willing or able to do so.

CareFirst – The Analysis of a working ACO Business Model
CareFirst will present an alternative commercial ACO model using small primary care “pods”, including virtual practices to bend the cost curve by creating accountability and shared savings.  CareFirst has 3009 primary care physicians and 300 additional nurse practitioners in 250 “pods” (note 89% of physicians are in groups of less than 4 MD’s) of which ½ are virtual.  65% of all their 3.5 million members are attributable to a primary care physician. Primary care providers are rewarded with significant automatic payment increases and have an opportunity for downstream shared savings.  Learn how this insurance company has worked to engage primary providers in delivering health care value.

Cost Reduction Without Cannibalization – How do we get there?
It’s not a question of “IF” but “WHEN” the draconian cuts to reimbursement will come, shaking the very foundation of how many current medical groups and health systems are built. Groups will have to find new ways to not only reduce overall costs of care but they will have to create a path to transition to whatever system is created.

What are your ideas for reducing costs without cannibalizing your medical group during this period of change?  What new partnerships or innovative business models seem attractive?  Members of the Council will be asked to share their ideas on how to get from here to there.
